15 Craigside offers lifetime residency with three levels of continuing care.
Independent Living
15 Craigside is Hawaii’s newest continuing care retirement community in
Honolulu. It is a moderate income retirement community. 15 Craigside offers a lifetime residency with a continuum of care that begins with independent living in one’s apartment with access to health care services, food, convenience services, personal assistance, if requested, security, maintenance, maid and linen service, numerous programs, activities and entertainment and, along the continuum, if needed, assisted living, skilled long term care and special care for those who develop Alzheimer’s disease and dementia.
Assisted Living
As part of 15 Craigside’s continuum, 15 Craigside will be a licensed provider for Assisted Living Services. Each apartment will be licensed so that, if needed, 15 Craigside can provide Assisted Living Services.
The purpose of the State’s regulation of Assisted Living is to allow residents to maintain an independent assisted lifestyle. Every resident initially receives a functional assessment which indicates if any type of services, including assisted living services or nursing services are needed. These functional assessments are updated annually, as required by regulation.
The initial and annual assessments are not “tests,” but rather a proactive program that enables a resident to identify needs and address ways to meet those needs. It also enables 15 Craigside and a resident to make informed judgments together with the resident’s physician and the resident’s family about what assistance or care is or may be needed.
Under the Assisted Living Facility license, medication management and/or assistance with certain activities of daily living are provided for residents in their apartments. The extent of individual’s need is based on a resident’s assessment and an agreed upon Service Plan.
The Health Care Center
The third major level of care in the continuum (Independent Living is the first level and Assisted Living is the second level) is Skilled Nursing and Intermediate Care (also referred to at times as Long Term Care) and Special Care for those with Alzheimer’s and dementia. Some residents may never need long term care, or for that matter, Assisted Living. Some may need only Assisted Living. Some may go through the entire continuum, and some may go from Independent Living directly to Long Term Care. Every person is special and different. Skilled nursing and related nursing services are provided to residents whose primary need is for 24 hours of skilled nursing care or nursing care, or special care for those with Alzheimer’s and dementia, or for hospice.
